Cheatham County families:
Please be on the lookout for a permission form that will be sent home with your child. This permission form is in response to the Family Rights and Responsibilities Act (FRRA) and Tenn. Code Ann. 63-1-173.
A new law has passed requiring parent permission for a minor student to be seen in the nurse's office for any reason. We have developed a permission form for you to indicate if you DO or DO NOT give permission for your student to receive basic minor care in the nurse's office.
Please note if the nurse assessment indicates anything other than minor care needed, the nurse/Cheatham County School District employee will call the parent for further consultation. This permission form is to indicate minor care can be given (i.e. band aids, gauze, ice packs, etc.).
If you do not wish to give permission, your child will be treated in emergency situations only and you will be called to care for your child any time your child needs to use the nurse’s office.
We recognize the importance of school nurses. All school nurses are professionally licensed and practice within the limits set forth by those licenses. Last school year, students returned to their learning environments 94% of the time after visiting the clinic.
Forms will be sent from each school by Monday, August 19. Please review, sign and return the permission form as soon as you can so there is no delay in care that is needed for your child. Thank you in advance for your help to keep students safe and healthy at school.
